Your not missing anything essential, and have described a limitation in analog signaling. What you need to think about is alternatives. There is always the call parking so that you push the caller into a state that totally frees you from them. You then are able to call the other extension and tell them the parked extension number. It doesn't allow for the 3 of you to conference in, but the call gets handed off well. If you get busy voicemail, you can hangup and retrieve the parked call yourself. Or you can ask, if they needed to speak to that person, they might be interested in leaving a voicemail.
