Generally it is because some one or some process did one or more of the following: 1. Updated firmware on the ATA 2. Updated software on the server 3. Changed a configuration setting 4. Let the smoke out of a chip AFAIK none of these devices or systems are using artificial intelligence, yet. Therefore when something just quits you need to start looking for what changed.
