Another problem might be alternate entry points in the dialplan. For example, spool files and the manager Originate command can both set up calls at a context, extension, and priority of their own choosing, rather than going through all steps of your dialplan (as external calls would). Also, SIP calls can setup to any extension in your entry context, including non-numeric extensions.
