I am successfully using this in my dialplan for a number of Sipuras
(modified to fit your dialplan):
exten => 700,1,SIPAddHeader(Alert-Info: info=<Bellcore-r2>)
Not saying there's no other way to get it accomplished, but this is known to
work (1.4.21.2).
