I had exactly the same problems as the first time, No audio (although on one attempt I had 1 way audio, but strangely the phone and the deskphone and other mobile appeared to be conferenced together), answering desk phone didn't answer call on mobile, hanging up didn't hang up etc. Although I think I'm using bluez-4.40, the USB dongle is also a CSR one like the OP, asterisk 1.6.1.1, with a nokia e61.
