We've talked about this on the VoIP users conference and the feeling is that most likely there is a human on the back end who is doing the transcription, if it is to be at all accurate. Anyone who has worked with the various speech-to-text software, such as ViaVoice or Dragon, knows that training is the key to really accurate transcription. That, and a good quality audio signal. The variations in audio quality you get in voicemail is probably too great to do this all with software only.
