So out of curiosity, if you were to limit the phone usage for an hour at the remote site to a max of 2 calls at a time, does the voice quality problem go away? If so, how many calls does it take to make it come back? That test will tell you if it's a bandwidth issue and how much of a bandwidth issue it is.
