Most people buy the codecs as software packages from one of a few companies that specialise in writing major DSP modules. The royalty those companies charge for the software usually includes the patent fees, which they pass on to the patent holders. If you are lucky, they will indemnify the equipment maker that they have paid all relevant charges. :-)
