By default (at least in 1.4X) voicemail.conf defines how the Asterisk server will send information to exchange or any other Mail server. The default usage is a simple "sendmail" command. If you want incoming stuff to be processed by asterisk, you would probably set up a daemon in c or some other language to process the information as a "local call".
