Has someone seen this? Is my Dahdi configuration wrong, or is it some
issue between Dahdi and CentOS?

I don't have much direct experience with the cards supported by the wctdm driver, but based on what you show here, it appears more like either a fundamental PCI bus incompatibility, the card isn't seated properly in the slot, or failed hardware.
