I glanced through the source for app_voicemail and am beginning to wonder if there need be a physical SIP device configured to use that mailbox for the mailbox to be polled. Is that the case? This Asterisk installation is acting as a VM server for a legacy phone system and none of the VMboxes are actually connected to a SIP phone on this box. Can this be the source of my problem?
