Okay, it seems to work if I add:
exten => 12345678,n,Set(CALLERID(num-pres)=allowed)
I guess if either of Name or Number aren't allowed then Asterisk
replaces the whole caller id string with the default:
"Anonymous" <sip:[email protected]>
