This was changed in v12.  There is a Hold and an Unhold event.
Events no longer have a subevent type field.  Instead there are
separate events.  e.g., DialBegin/DialEnd, Hold/Unhold,
MusicOnHoldStart/MusicOnHoldStop, etc.
