Yes, the schema can change between versions. Following the instructions on https://wiki.asterisk.org/wiki/display/AST/Setting+up+PJSIP+Realtime#SettingupPJSIPRealtime-InstallingandUsingAlembic will cause alembic to upgrade the tables.
