Is Are there any mysql-related module loaded?

Start with e.g.

  ldd /usr/lib/asterisk/module/cdr_mysql.so

Were the relevant modules built? Do you use direct MySQL support? ODBC?
Maybe you forgot to install development headers?

This one typically comes from the development headers package, so it's
probably installed.
