http://bugs.digium.com/bug_view_page.php?bug_id=0001086 is what I use to dynamically put in SIP entries. It works really well.
The extensions don't seem to be available in MySQL "officially" but I did find http://www.voip-info.org/tiki-index.php?page=Asterisk%20cmd%20DynExtenDB which I have not tried yet.
