I've used iaxclient with a USB headset, and it works fine.
I think the issue might be that many macs don't have microphone inputs for conventional headsets..

A friend of mine uses iaxcomm to connect to my *. The only problem is he
can not use a headset. I think it was because the devices are not
recognized or available...
