When I researched this issue a few months ago, I found a post from Mark Spencer saying that the dialed number was intentionally not sent to discourage people from setting up multiple IAXTel numbers for the same server. In a mailing-list discussion a month or so ago, somebody said that he was getting this information in the calls, but had to specifically request a configuration change by Digium.
