uniqueid was added long after the original cdr code was released, so it was added as a define to explicitly enable it. At some point it might end up enabled by default, but being backwards compatible by default is a good thing.
Regarding the userfield, I found it in the cdr_addon_mysql.c. It's a static variable controllable from the conf.
