Here's how I 'fix' my caller id for calls from FWD...

[fwd]
; routing for calls from FWD
exten => _X.,1,SetCidNum(*393${CALLERIDNUM})
exten => _X.,2,Macro(sipexten,1000,${KITCHEN},fwd)
...
