In my setup I have 2 of the TDM400P cards, with four FXO modules each.
I'm not having this problem on either of my TDM400 cards with a mix of FXO and FXS modules
Check that your impedence settings are correct, and failing that log a bug at bugs.digium.com with as much information as you can provide.
