Asterisk relys on mpg123 to play the music on hold.
Also according to my knowledge, asterisk needs clock to play on hold music.
The clock is provided by zaptel devices, if you don't have zaptel devices you need to 
compile ztdummy to provide clock to *.
I can't explain more because, it's as far as I know it ^_^
you can install mpg123 using ebuild
just emerge mpg123
to compile ztdummy, you need to do something to * source (please check the previous 
mail to know how to do that).
I have a zaptel device, that's why I don't need to compile ztdummy.
also music on hold in * doesn't rely on your sound-card.
