I added
> exten => _0.,1,Dial,Zap/1/${EXTEN:1}

to the extensions.conf
but I am not sure I follow you on the second part, do you want me to add
include => outgoing
to my sip.conf file?? I did both of these changes, and I still have the same
problem.
