Just FYI, It turned out that the problem was a simple one, but not something you'd expect to be a big issue these says (with ACPI etc): IRQ sharing. There was an onboard gfx card using IRQ 11, the same IRQ as the digium card.
lspci -v showed the true story, even though cat /proc/interrupts didn't. Hope that helps anyone else out there with similar issues. I think the idea here is to isolate wether the bad sound is due to your hardware and immediate PSTN switch or some farther away PSTN switch. Calls that go out of your Asterisk get routed one way in the PSTN jungle and calls coming to you probably get routed another way.
