Yes, I have seen the same behaviour. In my case it seems to be less than 
perfect dtmf tones. They are detected, but sometimes a very short break in 
the middle of a tone causes Asterisk to treat the rest of the tone as a 
new digit.

Not even the relaxdtmf has helped. I have access to a set of phones that 
triggers this behaviour. I'll submit a bug as soon as I have recorded the 
tones from within Asterisk.
