Everything looks ok in your Zapata.conf.  when I load your config on my
machine I can do 3 way calls and answer call waiting.  I'm thinking it may
be a problem in the extensions.conf.  Can you post what you have in the
extensions.conf for the extension (SIP phone) you are calling from the PSTN.
