Lol. It seems as simple as telling them to hit "9" and then dial. You can then get Asterisk to generate a dialtone, you could use something like DISA (I'm not too familiar with it [but I want to be]) or maybe just a background sound of a dialtone, and have it in a context where the outbound rules are.
I don't understand the need for a dialtone though, tell them it's more like a cell phone.
