where <MAC_ADD> is the MAC address of your phone (usually on a small sticker on the back of the phone), SERVER_IP_ADDRESS is the server ip address.
you'll also want to change: <loadInformation8 model="IP Phone 7940"></loadInformation8> <loadInformation7 model="IP Phone 7960">P0S3-07-3-00</loadInformation7>
to the match the firmware and phone model that you are using.
