I'm rather new to Asterisk, but experianced a similar problem. It turned out that I had a typo in my extension.conf file. Run the Asterisk cli <asterisk -r> and watch the diagnostic messages. Perhaps you will see what the problem is. If you have the vvvv option you should see the number that is passed to the ZAP.
