Do you have a [switch-2.nufone.net] entry in iax.conf?

[nufone]
type=peer
host=switch-1.nufone.net
username=myuser
secret=mysecret
qualify=500
context=NANPA

I place calls by Dial(IAX2/[EMAIL PROTECTED]/${EXTEN})

I think if you're dialing it direct (i.e. no entry in iax.conf) you need
to
say ${EXTEN:[EMAIL PROTECTED] (i.e., specify the context).

Just a guess.
