I've tried two with the same issue: 

Iax.oztralia.com
gw1.austechpartnerships.com

They're Australian providers so you're probably not familiar with them.

It's actually an external DSL modem but it is connected to the smoothwall
via eth0.  eth1 goes to the switch as you surmised.
This is actually a fairly standard configuration although more commonly used
in business environments.
