Another option is the AudioCodes Mediant 2000 devices. I have one with 8 T1/E1 ports, one with 4 ports, and one with 1 port. You can also get up to 16 ports, and there is a 2-port model as well. I am using SIP with them, but I think you can use H.323 with a different firmware.
I am using them in a different application than you, but they are standard SIP, and they seem to work great.
