I've been prefixing the callerID with asterisk then sending the call for
people that want this kind of service.

exten => s,1,Set(CALLERID(name)=S: ${CALLERID(name)})

would prepend an "S: " to the callerID name.

I know with the Sipura phones you can tell it what ring to play using the
INFO header of the call as well.  Check the voip-info wiki for stuff for
your phones, they all seem to play differently (which is why callerID was
the easiest for me to implement).
