My understanding is that area code actually matters for these items, but
that they're like the last bastian of 7 digit dialing.

I think that, for instance, I could order 310-1234 in Southern Ontario and
that another company could order 310-2100 in BC.  The reason is that a
customer at a previous job had a hard time securing a suitable 310 number in
all exchanges across Canada.  It was available in some but not others.

I also think that location is important.  For instance, if I were to dial
310-1234 in Toronto, it might be routed differently than if I were to use a
VOIP provider who dumped the call out in Ottawa (for reasons discussed
earlier).

Furthermore, and probably for these reasons, you _can_ dial the area code
for some if not all 310 numbers.  I know I've used it to call Emily at Bell
(ie 416-310-2355) on systems that were programmed to insist on 10 digit
dailing.   I'd be interested to know what would happen if you tried to dial
416-310-2355 from a phone in Montreal.  Is it allowed?  Is a toll call?

I can't answer all of your questions, but the dialing in my experience,
all you need to do is add the area code to that number and it works.

So I just set up a rule in my customer dial plan...

exten => _NXXXXXX,1,Goto(${CDR(accountcode):0:3}${EXTEN}|1)

So if they dial a 10 digit number, it just appends their area code to it
and then goes on normally.

I'm pretty sure, at the very least that they're Canada-wide, 
Panago<http://www.panago.com>for instance has 310-0001 in British Columbia, 
Alberta, Sask and Ontario.

I'd like to know how to get one too, so if you find out let us all know.
