Another way to isolate your editing to the extensisons_custom.conf might
be to create a custom context as the destintion for your DID.

Everyone before me is right...it's best (in [EMAIL PROTECTED]) to edit your 
config.
But I think you might want make sure that the context in it is well
pointed out...
One of the reason I think, u'r unable to troubleshoot this is probably
b/c your exten line has got a little typo in it...

exten => 14161234567,2,Dial(SIP/[EMAIL PROTECTED],,60,r )

After 111.111 you have 2 commas (,,) instead of one.....

Well, to be absolutely sure, do the following:
- call that DID and take a look @ your Asterisk Log.....
- try to ring an internal extension first.....something like exten =>
14161234567,2,Dial(SIP/200)
