Since there is no digital signalling or polarity change, it is difficult to "detect" the extensions has been answered, to the best of my knowledge.
However, you could use some of the intelligence of the follow-me app to play a message for the remote party to press "1" when the call has been answered, at which point the two parties could be connected. You could also play with the answering-machine detection scripts, they may have something you could use.
