Zoltan, My first guess (and it's only a guess) would be that you don't have enough upstream bandwidth on your internet connection. If you're running anything else on the connection and you don't have properly configured QOS on your router you're definitely going to have problems. Even having QOS is no real assurance you're going to get enough bandwidth.
By the way, you might consider removing the SPA 3102 from your VOIP connection and connecting asterisk directly to the voip provider. It's just adding another analog-digital/digital-analog conversion. Unless there's a reason to have it, it would seem simpler and easier to troubleshoot if you removed it. My only other though would be really bad silence supression on some device someplace that's trimming beginning and ends of words.
