I have seen this behavior only during a trixbox upgrade. The solution was to edit each user with the gui, ensuring voicemail is enabled, saving each user and then restarting asterisk. Don't know if that is the solution to your problem, but hope it helps. (And, yes, it is a rather windows-ie solution, but since it worked I never pursued it more).
