I haven't found a good way to do decent load balancing with asterisk. Asterisk still really seems to have this one box approach to design. I wish there was a decent way to create an asterisk cluster which would share dial plans, registrations, licenses, etc.
In terms of failover what we have been doing is deploying asterisk with things like heartbeat and drbd to keep two boxes in sync.
