You're in luck. Asterisk will do this automatically behind the scenes.

1. Define your SIP phone as you would expect in /etc/asterisk/sip.conf.
2. Define your IAX carrier in /etc/asterisk/iax.conf.
3. Create an extension macro to gain access to your iax line in
/etc/asterisk/extensions.conf.

This is what Asterisk does best. It does all conversion in the
background so you treat all channel types the same and just make the
call.
