Are you having this problem using a specific adapter or IP phone? Have you tried accessing the IVR using a different adapter or a soft phone to see if the problem still occurs?

A while back I asked about having DTMF issues specifically with an SPA-2102 adapter. I upgraded both the firmware and Asterisk (to 1.2.14 rev 48468) and the problem seemed to go away. It doesn't hurt to do some testing by calling a POTS lines and generating DTMF tones to hear exactly what is being heard on both sides.
