'qualify=yes' does a SIP OPTION ping every minute. One minute is not
enough for most of the routers.
20s seems to be a good value for keeping the NAT pinhole open for most
of the routers. Unfortunately, the 1 min timer value is not
configurable on asterisk.

firmware has a "SIP Proxy" setting which is on by default and completely
breaks normal SIP.
Once this was disabled SIP started working again but unfortunately the
router then stopped keeping state for UDP (SIP). The usual work-arounds
such as qualify=yes had no effect and no amount of tweaking of the
settings helped. Phones would simply become unreachable every 30 seconds
or so.
The device had to be swapped out and can now be added to the long list
of crappy firewall/routers that can't do VoIP.
