For commercial applications, I hear Leif's concern about support. It's not exactly 'off the shelf' but the Pfsense team now offers commercial support. At least now you can pay a bit of money, support the developers and know that you're going to get some help when things go awry rather than just having to go to the forums, ask nicely and hope someone is able to help you out.
