I think it depends where you are.... "outside the GTA" is a pretty
expansive area :)

I found Voice Network (voicenetwork.ca) has quite a number of
non-standard DIDs in areas like Sarnia and such (they were the first I
found to offer a Sarnia DID). I haven't used them extensively, but the
call quality seemed fine. It just takes a bit of time to provision a
DID (day or two) since the billing stuff seems to be manual by a
secretary, and not entirely automated.
