I've used the older nokia e61 and SIP works great. Configuration is a
nightmare. But once it's setup, it works like a regular extension.
When it's in range it connects to wifi, registers and can make/receive
calls.
The iPhone doesn't allow background processes - so you can't receive
calls unless you are running the SIP/IAX client app. So, it would be
more for outgoing calls only. (You might be able to jailbreak it to
run background apps, but i haven't done this.)
