Your provider submits the directory listing along with the port request and they become active at approximately the same time.
That's the "official" method. Yellow pages is completely different. They aren't officially aligned with any carrier. They pull "raw" listings from the 411 system and include them as a starting point but in theory you can get any number you want put in the Yellow pages if you are willing to pay. For example, you can put an Edmonton DID and business listing in the Toronto book if you want.
