The gateway is D-Link DIR-655. It doesn't support SIP ALG (that I can see), but I've stress tested them before and they hold the call well even when the link is full in either direction, but not sure what technology they are using for that.
Rogers came today, we plugged into that and it worked instantly, but I am still curious to understand more about the issue. I appreciate you effort and offer of help.
