I have a soft-only Asterisk PBX (1.6.2.9-2+squeeze2) running on Debian
Linux (2.6.32-5-xen-686) under Xen (xen-4.0-amd64) with a Linux dom0
(2.6.32-5-xen-amd64).

I realized lately that call audio has been very poor, but generally on
the IAX side I think (telco end). SIP (softphone) seems good.

Frequently when playing a soundfile in an extension it will suddenly die
in the middle of it and the caller hears no more audio. No errors are
seem on Asterisk. It's like IAX just goes kerplooie.

If you want, you can hear an example of the audio problem by calling
5192794018. Please don't leave a VM. It'll confuse people. :)

While writing this up I actually got an error after it stopped:
-- <IAX2/5192794018-1286> Playing
'/var/spool/asterisk/voicemail/def/5192794018/temp.slin' (language 'en')
[Jul 5 18:11:07] WARNING[1909]: chan_iax2.c:1094 jb_warning_output:
Resyncing the jb. last_delay -51, this delay 2043, threshold 1002, new
offset -2043

followed by a few more seconds of audio, then deadness again.

So this points to network I think.

After turning iax2 debug on:
-- <IAX2/5192794018-2137> Playing
'/var/spool/asterisk/voicemail/rmf/5192794018/temp.slin' (language 'en')

Audio died around 20s in.

Where do I go from here?
