I spent a lot of years with a dial-up ISP. We got rid of the secondary D channel because there was no logical reason for it in this day and age. The chance of your D channel dropping and the other timeslot still working correctly are damn near zero. In our application the extra voice channel was worth MUCH more than any pie- in-the-sky notion of failover.
