I have been pleased with the old EXGN service (recently merged and took on
another name (Vitelity), but can still be reached at http://www.exgn.net/ ).
They do IAX and SIP, and have been low-priced and reliable in my experience.
Took me quite a while to get a custom DID after I ordered it, though
provisioning is pretty much instant if you pick from the pool of numbers they
have available.
Anyway, there are my $0.02. I don't work for or benefit financially from
my reccomendation, just wanted to pass it on since you asked.
